OBJECTIVE@#To evaluate the early effectiveness of a new minimally invasive plate in the treatment of varus-type ankle arthritis.@*METHODS@#A clinical data of 15 patients with varus-type ankle arthritis who met the selection criteria between March 2021 and October 2021 were retrospectively analyzed. All the patients were treated with medial open-wedge supramalleolar osteotomy and fibular osteotomy. The osteotomies were fixed with the new minimally invasive plate. There were 7 males and 8 females with an average age of 49.8 years (range, 16-71 years). The causes of ankle arthritis included post-fracture deformity in 1 case, sprain in 8 cases, and acquired clubfoot in 1 case; and 5 cases were without obvious factors. The disease duration ranged from 1 to 12 years, with an average of 4.1 years. Comparisons were made between pre-operation and the last follow-up in the Takakura staging, the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot score, foot function index (FFI), visual analogue scale (VAS) score, tibial anterior surface angle (TAS), tibial lateral surface angle (TLS), and talar tilt (TT).@*RESULTS@#All incisions healed by first intention. All patients were followed up 7-18 months (mean, 12.8 months). At last follow-up, the AOFAS ankle-hindfoot score, FFI, VAS score, and Takakura staging significantly improved when compared with the preoperative ones ( P<0.05). X-ray films showed that the osteotomy healed at 3 months after operation. At last follow-up, TAS significantly increased and TT decreased when compared with the preoperative ones ( P<0.05), and the difference in TLS between pre- and post-operation was not significant ( P>0.05). Complications included 1 case of intraoperative screw breakage and 2 cases of nerve injury of the affected foot. None of the patients complained of significant discomfort at the plate placement during follow-up, and no loosening of the internal fixator occurred. Eleven patients were very satisfied with the effeectiveness, while 4 were relatively satisfied.@*CONCLUSION@#The new minimally invasive plate for the varus-type ankle arthritis has good early effectiveness in relieving ankle pain, correcting deformity, improving limb alignment and ankle function, and reducing the incidence of postoperative incisional complications.

RESUMO

Communication with the family members of donors is an integral part of the organ donation and transplantation, and the core of it lies in building trust through interpersonal communication. Every word and deed from the communicator will directly affect the overall impression of family members of potential donors towards organ donation. Regardless of whether or not granted the donation ultimately, family members may share their personal experiences and feelings with friends and relatives around them, which develops a secondary dissemination. Therefore, "the choice of best candidate for communication with family members of organ donation" has been an issue that organ donation practitioners have been working on in clinical practice. Taking into consideration of the experiences from different countries or regions, various advices and practices on this issue have been proposed due to differences in social systems, cultural background, organizational structure, clinical practice, etc. In this paper, we had a discussion on this topic, summarize the difficulties currently encountered in communication with family members and propose corresponding strategies.

Cholangiocarcinoma is a primary malignant tumor derived from the epithelium of the intrahepatic and extrahepatic bile ducts, and vasculogenic mimicry induced by hypoxia and lack of nutrition is a key factor for malignant proliferation, recurrence, and metastasis of cholangiocarcinoma. Previous studies have shown that autophagy maintains cell nutrition metabolism under the condition of a lack of nutrition, and preliminary experiments have confirmed that autophagy was associated with VM in cholangiocarcinoma and there was high expression of PAT4 in cholangiocarcinoma cells; on the basis of these studies, it is pointed out that in cholangiocarcinoma, protective autophagy regulates VM formation by maintaining intracellular metabolic balance and cellular homeostasis. As a nutrient sensor in tumor microenvironment, PAT4 mediates protective autophagy via the PI3K-Akt-mTORC1 signaling pathway to regulate VM formation. Histological, cellular molecular, and in vivo experiments have confirmed that autophagy regulates VM formation by maintaining cell metabolism, stem cell features, and extracellular matrix remodeling, which helps to investigate the signal mechanism for PAT4 mediating autophagy to regulate VM. It is suggested that autophagy is the source of energy and nutrition in cholangiocarcinoma cells under the condition of a lack of nutrition, and PAT4 is the trigger point for autophagy in regulating VM formation. These findings provide new thoughts for the metabolism of cholangiocarcinoma cells and lay a theoretical foundation for antiangiogenic drugs combined with autophagy inhibitors in the treatment of highly aggressive tumors.

Objective To investigate the effects and related risk factors of different vascular access types on new atrial fibrillation in maintenance hemodialysis (MHD) patients.Methods This was a single-center prospective cohort study.Patients who established long-term dialysis access and were voluntarily followed up in the Second Hospital of Tianjin Medical University from January 1,2013 to June 30,2013 were enrolled to follow-up for 5 years.Patients were divided into fistula group (patients with autogenous arteriovenous fistula) and catheter group (patients with tunneled cuffed internal jugular vein catheter).The incidences of new atrial fibrillation in the two groups were compared by Kaplan-Meier survival analysis.Cox regression analysis and receiver operator characteristic curve (ROC) were used to assess the risk factors of new atrial fibrillation.Results A total of 315 eligible patients were enrolled,including 150 males (47.62%).There were 189 patients (60.00%) in the fistula group,and 126 patients (40.00%) in the catheter group.Multivariate Cox regression analysis showed that older age (HR=1.021,95%CI 1.003-1.040),arteriovenous fistula (HR=1.899,95%CI 1.019-3.539),increased dialysis blood flow (HR=1.030,95%CI 1.010-1.051) and left atrial diameter (HR=1.097,95%CI 1.022-1.177) were independent risk factors for new atrial fibrillation in MHD patients (all P < 0.05).Kaplan-Meier survival analysis showed that the incidence of new atrial fibrillation in fistula group was higher than that in catheter group (Log-rank A2=9.53,P=0.002).ROC curve analysis showed that age [the area under the curve (AUC)=0.608,P=0.008],arteriovenous fistula (AUC=0.594,P=0.021),dialysis blood flow (AUC=0.659,P<0.001) and left atrial diameter (AUC=0.604,P=0.011) could predict the occurrence of new atrial fibrillation.Condusions Older age,arteriovenous fistula,increased blood flow during dialysis and left atrial diameter are independent risk factors for new atrial fibrillation in MHD patients,which can predict the occurrence of atrial fibrillation.The incidence of new atrial fibrillation in patients with arteriovenous fistula is higher than that in patients with catheter.

Objective:To investigate the influence of arsenic troxide (As2 O3) and γ-secretase inhibitor MW167used alone or in combination in the drug resistance of HepG/ADM cells,and to clarify its mechanisms.Methods:The HepG2/ADM cells were treated with different concentrations of As2 O3 (0.25,0.50,1.00,2.00,4.00,8.00mg· L-1) andMW167 (10,20,40μmol· L-1) for 24,48 and 72h,and MTT method was used to detect the optical density (A) values,then the inhibitory rates of proliferation were calculated and the drug concentration and treatment time were confirmed.The HepG2/ADM cells were divided into blank group,As2O3group,MW167 group and combination group;the non-cytotoxic doses of As2 O3 and MW167 according to the results of MTT were chosen and the HepG2/ADM cells were intervened for 48 h;the apoptotic rates were detected by FCM;RT-PCR and Western blotting methods were employed to detect the mRNA and protein expression levels of Notch-1,Hes-1,MDR-1 (P-gp),Bcl-2 and Bax in the cells in various groups.Results:At the same concentration,the inhibitory rates of proliferation of HepG2/ADM cells were enhanced with the prolongation of the treatment time of As2O3 and MW167 (P<0.05 or P<0.01);at the same time point,the inhibitory rates of proliferation of As2O3 and MW167 in the HepG2/ADM cells were increased with the increasing of drug concentration (P<0.05);the non-toxic doses of As2O3 and MW167 were 0.25 mg · L-1 and 10 μmol · L 1 and the intervention time was 48 h.After treatment for 48 h,compared with blank group,the apoptotic rates in various intervention groups were increased (P<0.05),especially in combination group (P<0.01);the expression levels of Notch-1,Hes-1,MDR-1 (P-gp),Bcl-2 mRNA and protein in various intervention groups were lower than those in blank group (P<0.05),especially in combination group (P<0.01);compared with blank group,the expression levels of Bax mRNA and protein in various intervention groups were increased (P<0.05),especially in combination group (P<0.01).Conclusion:As2 O3 can reverse the drug resistance of HepG2/ADM cells,its mechanism may be related to inhibition of cell proliferation,promotion of apoptosis,down-regulation of the expression levels of Notch-1,Hes-1,MDR (P-gp),Bcl-2 and up-regulation of the expression levels of Bax mRNA and protein.

Objective To investigate the clinical value and safety of cedilanid, esmolol in the treatment of atrial fibrillation. Methods From August 2014 to August 2016 our Hospital from 117 patients with atrial fibrillation clinical data, according to the random number distribution principle, the patients were divided into observation group 59 cases and control group of 58 cases, all patients were given the treatment of primary disease, the clinical symptoms, the patients in the observation group were given oxygen, has given furosemide, cedilanid, after micro injection pump intravenous nitroglycerin, 5-20 g/min. Start after the injection of nitroglycerin, establish another vein channel, every 30 min to 0.2 mg patients, the treatment group were treated with intravenous injection of small dose esmolol. The clinical efficacy, ventricular rate, systolic blood pressure, diastolic blood pressure and adverse reactions were observed in two groups. Results The early and late effective rates of the two groups were not significantly different. The observation showed that after treatment, the ventricular rate, systolic pressure and diastolic pressure in the observation group were significantly higher than those in the control group (P<0.05), and the incidence of adverse reactions in the two groups was significantly different (P<0.05). Conclusion High dose cedilanid combined with small dose esmolol in treatment of atrial fibrillation, obvious curative effect, high safety, can choose the appropriate application.

<p><b>OBJECTIVE</b>To systematically evaluate the efficacy and safety of electroacupuncture (EA) for post stroke cognitive impairment (PSCI).</p><p><b>METHODS</b>The randomized clinical trials (RCTs) regarding EA for PSCI published before October of 2016 were researched in China National Knowledge Infrastructure (CNKI), Chinese Biomedical Database (CBM), WanFang database, VIP medicine information system, PubMed and Cochrane Library. The literature screening and information extraction was conducted by two independent reviewers. The quality assessment was performed based on the guidance of the Cochrane Reviewers' Handbook, and Meta-analyses was performed by using RevMan 5.3 software.</p><p><b>RESULTS</b>Totally 14 RCTs were included, involving 896 PSCI patients. The results of Meta-analyses showed the EA group was superior to the control group in improving the MMSE [=1.78, 95%(0.24, 3.32),=0.02], the MoCA [=1.92, 95%(0.96, 2.88),<0.000 1], P300 latency [=-11.01, 95%(-18.91, -3.11),=0.000 6], P300 amplitude [=1.56, 95%(1.14, 1.98),<0.000 01], FMA score [=10.74, 95%(2.67, 18.82),=0.009] and the clinical effective rate [=1.37, 95%(0.98, 1.91),=0.06]. However, the comparison of BI score in both group had no significant differences [=6.38, 95%(-2.41, 15.18),=0.15].</p><p><b>CONCLUSION</b>This Meta-analyses confirmed EA is effective and safe for PSCI, which could improve cognitive function and motor function. However, because of low quality of the included studies, more well-designed multicenter RCTs are needed.</p>

Objective To observe the effect of Natural mountain spring on oxidative damage of aging mice . Methods Thirty male aging mice were randomly divided into experimental group ( drink Natural mountain spring ) and control group ( drink tap water ) according to the level of MDA .The serum in the two groups was taken for T-SOD activity and MDA content analysis after two months .Results The MDA content in experimental group decreased markedly than that in control group .Conclusion Natural mountain spring could alleviate oxidative damage by free radicals to extent .

Objective To observe the effect of electric acupuncture on the infarct volume and amount of cerebral cortex and spinal neuron at different times of cerebral ischemia/reperfusion(I/R)in stroke prone renovascular hypertensive rats(RHRSP)with middle cerebral artery occlusion(MCAO),and investigate the possible mechanisms of electric acupuncture on remote damage in ischemic stroke. Methods 480 male SPF Sprague-Dawley(SD)rats were duplicated to form the RHRSP models by clamping both kidneys. 370 successful ones were selected by taking the tail artery blood pressure,and divided into hypertension group and sham operation group(each n=60)by random number table method. The MCAO models were created by stringing middle cerebral artery in the remaining RHRSP. The nerve function defect score(NDS)was graded by Longa 5 point method after the rats waked up from anesthesia,then the ones scored 1-3 were enrolled. Totally,there were 190 rats with MCAO successfully created from which 10 were randomly selected to determine the infarct size by 2,3,5-triphenyl four azole nitrogen chloride(TTC)staining. The remaining 180 MCAO rats were randomly divided into model group,electric acupuncture group and fake acupuncture group(each n=60). The sham operated group only received surgical trauma;the electrical acupunctures atBaihuiandDazhuiacupoints on Du channel were performed on the day of model establishment in electric acupuncture group,once a day for 28 days;in fake acupuncture group,sticked the acupuncture needles at the skin ofBaihuiandDazhuipoints,then gived the same electrical acupuncture treatment. On 1,7,14 and 28 days after treatment,the rats of each group were respectively sacrificed,and the brains were collected,then the infarct volume and spinal neuron number were calculated by Nissl staining. Results ①Cerebral infarction volume:No infarcts were found in hypertension group and sham operated group. On 1 day and 7 days after MCAO,the infarct volumes were increased gradually in model group,electric acupuncture group and fake acupuncture group〔infarct volumes on 1 day were(12.36±0.11)%, (12.19±0.15)%,(12.24±0.16)%,and on 7 days were(20.01±0.24)%,(19.54±0.61)%and(19.77±0.25)%, respectively〕,and on 14 days and 28 days after MCAO,the infarct volumes were decreased gradually〔infarct volumes on 14 days were(17.18±0.23)%,(16.96±0.11)%,(17.08±0.62)%,and on 28 days were(14.38±0.21)%, (13.95±0.46)% and(14.29±0.37)%,respectively〕,but the differences among three groups had no statistical significance(all P>0.05). ② Cortex area:On 1 day after MCAO,the differences in the cortical neuron numbers among all groups were not statistically significant(all P>0.05). On 7,14,28 days after MCAO,the cortical neuron number of sham operated group was more than that of hypertension group,but the difference had no statistical significance(all P>0.05). Compared with sham operated group,the cortical neuron number in model group began to increase significantly after 7 days;compared with model group,the cortical neuron number in electric acupuncture group was increased obviously(cell/HP,7 days:75.48±2.41 vs. 68.78±1.42,14 days:61.32±2.60 vs. 48.78±1.41,28 days:53.65±1.46 vs. 28.78±1.21,all P0.05).③Spinal cord area:On 1,7,14 days after MCAO, the differences of the spinal cord neuron numbers among all groups were not statistically significant(all P>0.05). On 28 days,compared with hypertension group,the cord neuron number of sham operated group was increased,but the difference had no statistical significance(P>0.05). Compared with model group,the cord neuron number in electric acupuncture and fake acupuncture groups was inecreased(cell/HP:21.32±1.60,16.17±1.05 vs. 15.02±1.18),the difference being statistical significant in electric acupuncture group(P0.05). Conclusions Generally,the secondary spinal(cervical part)neuron death occurs after cerebral infarction in rats. The therapeutic action of electric acupuncture may reduce the secondary spinal neuron damage at remote site after cerebral infarction,that is possibly the mechanism of electric acupuncture for the protection of brain in hypertensive rats from I/R injury.

Objective To investigate the effects of different doses of gingko flavonoid on the expression of nuclear factor-κBp65 (NF-κBp65) and tumor necrosis factor-α(TNF-α) in mice with non-alcoholic fatty liver disease(NAFLD) .Methods The mice mod-el of NAFLD was constructed by the high fat diet for 4 weeks .The mice were randomly divided into the control group ,high fat model group ,high ,middle and low doses of gingko flavonoid groups .High ,middle and low doses of gingko flavonoid groups were respectively gavaged with the different doses of gingko flavonoid .The normal control group and the high fat model group were ga-vaged with normal saline .After 12 weeks ,the mice were killed for taking liver tissue and detecting NF-κBp65 expressions ,and ser-um TNF-αand triacylglycerol(TG) levels were detected .Results Compared with the normal control group ,the levels of NF-κBp65 ,TNF-αand TG in the high fat model group were increased(P<0 .05) .Compared with the high fat model group ,the levels of NF-κBp65 ,TNF-αand TG in the high ,middle and low doses of gingko flavonoid groups were decreased (P<0 .05) ,moreover the higher the dose ,the more obvious the decrease .Conclusion Gingko flavonoid may play certain role in the treatment of NAFLD by reducing the generation of NF-κBp65 ,TNF-αand TG .

Objective To establish an accurate sorting method with less interference to the cell activity for isolating CD 3+CD4+T lymphocytes in human intestinal tissue .Methods Firstly the digestive enzymes were adopted to decompose the human intestinal tissue ,and then the cell suspension was obtained by the filtration of 40 μm cell strainer .Mononuclear cells were collected by using continuous density Percoll gradient centrifugation technique ,then the flow cytometry was adopted to sort CD3+ CD4+ T lympho-cytes in mononuclear cells .The survival rate ,cell purity and morphological observation of the sorted cells were adopted to conduct the evaluation on the sorting method .Results The digestive enzymes well decomposed human intestinal tissue into cell suspension , the purity of CD3+CD4+ T lymphocytes by using continuous density Percoll gradient centrifugation was (43 .9 ± 7 .3)% ,while the purity of CD3+CD4+ T lymphocytes was(96 .9 ± 1 .2)% after fluorescence-activated cell sorting ,the difference between them was statistically significant(P<0 .01) .The survival rate of sorted CD3+CD4+ T lymphocytes was(97 .8 ± 1 .6)% with intact cell shape . Conclusion The Percoll gradient centrifugation technique combined with the fluorescence-activated cell sorting not only collects highly pure human intestinal CD3+CD4+ T lymphocytes with little influence on the cell activity and shape ,but aslo may be carried out under sterile conditions ,the sorted cells can continue to be used in the other functional researches .

Objective To investigate the influence of arsenic trioxide (AS2 O3 )in the vasculogenic mimicry (VM ) of HepG2 cells, and to preliminary clarify the possible mechanism of inhibition of AS2 O3 on the VM. Methods Themean inhibitory concentration (IC50 )of AS2 O3 72 h after treatment of HepG2 cells was calculated by CCK-8 assay.The HepG2 cells were cultured on 3-D Matrigel and randomly divided into control group, 1/2 IC50 AS2 O3 group and IC50 AS2 O3 group.IPP software was used to calculate the number,length and area of VM,and the expression levels of VM-related proteins VE-cadherin and MMP-2, apoptotic-related protein caspase-3 and proliferation-related protein PCNA were detected by Western blotting method.Results The IC50 of AS2 O3 was 10μmol·L-1 72 h after treatment of HepG2 cells.The number,length and area of VM in 1/2 IC50 and IC50 AS2 O3 groups were significantly lower than those in control group (P0.05).Conclusion AS2 O3 can inhibit the forming of VM of HepG2 cells,which indicated that its mechanism may be related to inhibiting the expressions of VE-cadherin and MMP-2 .

RESUMO

Refractory central nervous system (CNS) lymphoma in patients with non-Hodgkin lymphoma (NHL) carries a poor prognosis, with a median survival of 2-6 months. CNS involvement in NHL is associated with young age, advanced stage, number of extranodal sites, elevated lactate dehydrogenase, and international prognostic index (IPI) score. The most promising treatment of autologous stem cell transplant can extend median survival from 10 to 26 months. CNS prophylaxis is required during the initial treatment of NHL subtypes that carry a high risk of CNS relapse, such as Burkitt lymphoma (BL), and lymphoblastic lymphoma.The use of CNS prophylaxis in the treatment of diffuse large B-cell lymphoma is controversial because of the low risk of CNS relapse (≈5 %) in this population. The risk models that aim to identify predictors of CNS relapse in NHL.

Objective To explore the effect of ventilator circuit change frequency on the incidence of ventilator-associated pneumonia (VAP). Methods Patients receiving mechanical ventilation in the ICU,Department of Emergency, Respiratory Department and Department of Neurosurgery from March 2008 to September 2010 were randomized into two groups. For these two groups ,the ventilator circuit was changed once or twice a week. The recorded parameters included the clinical symptoms and signs of the ventilated patients. Samples at different parts of the circuit were collected for microbiological detection. The data were analyzed statistically. Results The incidence of VAP was 28. 30% ( 13/53 ) in twice-a-week group and 35.84%( 19/53 ) in once-a-week group. There was no significant difference between the two groups. The rates for positive microbiological detection in the circuit were 48. 16% and 44. 49% for once-a-week and twice-a-week group,respectively. No significant difference was observed ( P > 0.05 ). Moreover, there was no significant difference in terms of the microbiology positivity between different parts of the circuit(P > 0. 05 ). Gram-negative bacteria were the main pathogen of VAP with Acinetobacter baumannii ranking at the top. Conclusion Frequency of Ventilator circuit change does not influence the incidence of VAP. We suggest that the frequency for ventilator circuit change should be once a week. At the same time, the nurse staff should be trained for specific technology and the incidence of hospital infection should be controlled at multiple rings of the chain.

Mantle cell lymphoma is included in the World Health Organization classification as distinct lymphoma subtype characterized by the t(11;14)(q13;q32) translocation,which results in overexpression of Cyclin D1.The clinical presentation often includes extranodal involvement,particularly of the bone marrow and gut.The prognosis of patients with mantle cell lymphoma (median overall survival,3-5 years) is poorest among B-cell lymphoma patients,even though a prospectively difficult to identify subgroup can survive for vears with little or no treatment. Conventional chemotherapy is not curative but obtains frequent remissions (60%-90%) which are usually shorter (1-2 years) compared with other lymphoma entities.Very intensive regimens,including autologous and allogeneic stem cell transplantation,seem required to improve the outcome,but with the median age of diagnosis being 60 years or more,such approaches are feasible only in a limited proportion of patients.The possibility of treating patients based on prognostic factors needs to be investigated prospectively.

RESUMO

Since its discovery as the first human tumor associated virus, Epstein-Barr virus (EBV) has been implicated in the development of a wide range of B-cell lymphoproliferative disorders, including Burkitt' slymphoma, classic Hodgkin' s lymphoma and lymphomas arising in immunocompromised individuals (posttransplant and HIV-associated lymphoproliferative disorders). T-cell lymphoproliferative disorders that have been reported to be EBV associated include a subset of peripheral T-cell lymphomas, angioimmunoblastic T-cell lymphoma, extranodal nasal type natural killer/T-cell lymphoma, and other rare histotypes. EBV encodes a series of products interacting with or exhibiting homology to wide variety of antiapoptotic molecules,cytokines, and signal transducers, hence promoting EBV infection, immortalization and transformation.However, the exact mechanism by which EBV promotes oncogenesis is still an area of active debate. This review is focused on the pathology, diagnosis, classification, and pathogenesis of EBV-associated lymphomas.Recent advances in EBV cell-based immunotherapy, which is beginning to show promise in the treatment of EBV-related disorders, are discussed.

The spectrum of renal lesions that was seen in patients with myeloma include myeloma kidney, or cast nephropathy; light chain (AL) amyloidosis; monoclonal Ig deposition disease(MIDD); and less frequently, cryoglobulinemic glomerulonephritis and proliferative glomerulonephritis.ln native renal biopsy studies of patients with myeloma and renal disease, 40 % to 63 % had east nephropathy, 19 % to 26 % had light-chain deposition disease, 7 % to 30 % had amyioidosis, and 1% had eryoglobulinemie renal disease. The renal pathology of cast nephropathy, MIDD, and amyloidosis was diverse.Extrarenal manifestations of MIDD, as with amyloidosis, were frequent, the incidence of renal insufficiency and ESRD was high.The morbidity and mortality were a significant increase.Patient and renal survival were significantly worse in patients with coexisting nephropathy.Therapy of renal lesions with myeloma was similar to that for multiple myeloma and consisted of chemotherapy alone,and treated renal insufficiency with hemodialysis and peritoneal dialysis, and used of high-dosage chemotherapy followed by ASCT to reduced the level of light-chain production.

RESUMO

OBJECTIVE To promote the multidrug-resistant bacteria infection control. METHODS Questionnaire investigated has been used to understand what degree the healthcare workers have known about controlling multidrug-resistant bacteria infection,what difficulties they would be met when they had put prevention and control measures in practice. RESULTS Total 13 questions of prevention and control multidrug-resistant bacteria nosocomial infection,the rate of right answer was 26.9% and the rate of partial right answer was 38.4%.The healthcare workers had lacked of professional knowledge about the prevention and control of multidrug-resistant bacteria nosocomial infection. The major difficulties were crowded wards,short of beds,incomprehension of patients and their families,cost of articles for disinfection and isolation when the medical staff put isolation measures into practice. CONCLUSIONS All prevention and control measures againt multidrug-resistant bacteria infection should be put into practice. We should strengthen training of healthcare workers about professional knowledge,gain support of leaders,and enhance communication between medical staff with patients and supervision.

RESUMO

Involvement of the central nervous system(CNS)in multiple myeloma(MM)is very uncommon.and it has been observed in approximately 1% of the MM patients.This review summarizes the clinical and laboratory characteristics and treatment modalities of 109 patients with CNS myelomatosis(CNS MM)reported in the literatures.CNS MM has a wide spectrum of neurological symptoms and signs.No guidelines for therapy of CNS MM are available,which has resulted in a large variation in the treatment schedules.Treatment options include intrathecal chemotherapy(IT),systemic chemotherapy(SC),cranial irradiation(CI)or a combination.The prognosis of CNS MM remains poor,with an overall median survival from the time of diagnosis to death of 2.0 months(range 0.1-25 months).Patients who were treated with CI had a significantly(P=0.004)longer survival time when compared with patients without CI.

Objective To observe the effect of Vascular Endothelial Growth Factor(VEGF) to the expression in hepatoma cells affected by TGF-?1 and hopxia. Methods HepG2 cells were cultured in vitro, and treated with different doses of TGF-?1 and Cobalt chloride hexahydrate(CoCl2), or with TGF-?1 and CoCl2. The change of VEGF protein and mRNA was detected by immunohistochemistry and semiquantitative reverse transcriptase-polymerase chain reaction (RT-PCR). Results Immunohistochemistry and RT-PCR showed that the expression of VEGF protein and mRNA were significantly higher in TGF-?1 groups or CoCl2 groups than that of control group(P
